The Help Me Grow Transition Process

Help Me Grow will provide services for children until they reach the age of 3 years old, but this does not mean that all programs and support for you and your child will end then.

Approximately 180 days before your child turns 3 years old, your OFY Service Coordinator will discuss options available to you once your child turns 3 years old. A Transition Goal will be established to help you identify the steps to move your child from HMG services to the most appropriate program available to you in the community.

Your OFY Service Coordinator acts as your liaison to help set up meetings between you and identified programs to meet your child’s needs. These programs may include: public or private preschools, Head Start, child care centers, etc.

Your OFY Service Coordinator is there to help you “navigate the system.” We want to help you consider all of the options your child is eligible for and guide you in choosing the one that best supports your child and family.
