If your child is eligible for Part C Services, your OFY Service Coordinator will setup a meeting to establish a plan and set goals for your child and family to reach. This plan is called the Individual Family Service Plan or IFSP.
If certain therapies are recommended (such as speech, physical, or occupational therapy), your OFY Service Coordinator will assist you in locating and accessing these services. The OFY Service Coordinator also searches for other resources in the community that could be helpful to both you and your child. Your OFY Service Coordinator acts as a liaison between you and other providers of service whenever necessary. The services your child receives are then monitored by you and the OFY Service Coordinator. Regular reviews of your IFSP are then set up. You and your team of service providers can discuss progress on goals and create new goals when appropriate.
