Does Your Child Need Help Me Grow Early Intervention?

Your OFY Service Coordinator will complete the process of assessing your child within 45 days of your child’s referral to Help Me Grow. Much is involved. Your child will receive screenings, assessments, and evaluations. These will be done by your Service Coordinator, yourself, and qualified professionals in order to determine if your child meets Part C eligibility requirements. Your child will receive:

• Vision Screen.
• Hearing Status Check.
• Nutrition Screen.
• Social-Emotional Screen.
• An assessment/evaluation in five separate developmental areas (domains).

A written report will then be prepared and shared with you. Your team will discuss any recommendations that are being made. At this time you will be informed if your child is eligible for Part C Services.
