Adoption Service Fees

| Home Study | $1,300 |
| Post-Placement Services1 | $150 per visit ($50 extra charge for each additional sibling) |
| Large Family Assesment2 | $450 |
1. Post-placement services can include matching, meeting attendance, post-placement visits, reports, preparation of JFS01699 form, court appearance, etc.
2. Large Family Assessment is required by the Ohio Department of Job and Family Services for a family that will have five or more children after the placement of a potential adoptive child.
Collection of Adoption Fees
Fees for the Application, Home Study, and Large Family Assessment (if necessary) will be collected as follows:
- One-half is due with the completed application.
- One-half is due at the completion of a typed and approved home study document.
- Payment must be made before the home study will be released.
- Collection of fees for post-placement reports:
- Payment is due prior to or at each home visit. Payment must be made before the report will be released.
Any other arrangement regarding fees or for collection of fees must be made with the Executive Director.
