Help Me Grow Part C Early Intervention Services

Help Me Grow (“HMG”) Part C Services are for children who:

  • Are under the age of 3 years old, AND
  • Have a specific need that your family doctor identifies.

Your doctor can refer you to Help Me Grow. However, if you are a parent who has some specific concerns about your infant/toddler’s development, you can also call Help Me Grow directly. The number to call is 216-698-7500. Ask to talk to someone about your concerns regarding your child’s development. Help Me Grow will then link you to an agency that provides “service coordination.” One of those agencies is us, Options for Families and Youth (“OFY”). 

An OFY Service Coordinator will be assigned to your family to be a resource person to assist and guide you. Service Coordination is free to eligible families that meet the federal guidelines. See Assessing Your Child for the next steps.

OFY Help Me Grow Part C Service Coordination is also provided to families whose children:

  • Are under 3 years old, AND
  • Have identified developmental delays OR
  • Have a medical diagnosis identified by ODH as having a high probability of resulting in a developmental delay.

OFY works with the William Patrick Day, East Cleveland Early Intervention Centers and all of their affiliated sites on the east and west sides of Cleveland. We are members of a team that includes the school’s teachers, assistants, therapists, children, and their families. OFY Service Coordinators assure that families are getting what they need in terms of help and resources for their children.

Having a child with special needs can be demanding, frustrating, and confusing. Your OFY Service Coordinator is someone who will be reliable and stay with you during your child’s early years. They will help you with:

  • The process of assessing your child
  • Determining eligibility
  • Locating services and resources
  • Ultimately transitioning your child into the best setting for preschool.
